Summary:
Invasive electrical neurostimulation may be used for patients with neuropathic pain who do not respond to or suffer from side-effects from other therapies, eg pharmacotherapy and TENS. The most wide-spread method is spinal cord stimulation (SCS) which annually is applied to more than 500 Scandinavian patients. Around 60-75% of neuropathic pain cases enjoy so beneficial effects of SCS that they desire permanent implants after a test period.
Recently randomized controlled studies have clearly demonstrated that SCS is superior to conventional modes of therapy in chronic neuropathic pain conditions, such as persistent pain after spine surgery and complex regional pain syndromes (CRPS type 1). The great advantages is that the technique is minimally invasive, reversible and has few serious side-effects.However, the initial cost is fairly high but follow-up studies show that this cost is regained after between 2-3 years.
