Summary:
The Swedish Council on Technology Assessment in Health Care (SBU) has conducted a systematic review of the empirical support for dialectical behavior therapy in the treatment of borderline personality disorder. The target outcome variables were parasuicidal acts (self-inflicted harm) and drug abuse. Borderline personality disorder is characterised by a pattern of problems in the regulation of emotions, lack of impulse control, intense and unstable interpersonal relationships and low self-esteem. Self-damaging behaviors and suicide attempts are common. Dialectical behavior therapy is a cognitive behavioral treatment that has specifically been developed for suicidal persons with borderline personality disorder. The method involves behavioral techniques such as behavior analysis and methods to foster acceptance of feelings. The effects of dialectical behavior therapy has been compared with other standard psychiatric treatments (treatment as usual). On the basis of 6 identified randomized trials SBU concludes that there is promising but limited support for dialectical behavior therapy and that the method should be evaluated in Swedish settings. Moreover, there is limited information on the cost-effectiveness of dialectical behavior therapy.
